These can be obtained by calling to request an Intake Packet.
Insurance
Currently, we are only accepting Blue Cross Blue Shield (BCBS) and State of Mississippi (AHS) Insurance plans. If you have coverage with another insurance provider, as a courtesy we will provide a receipt with which you can file with your own insurance plan and possibly be reimbursed a portion of your session fee. If you have any questions regarding insurance, fees, balances or payments, please feel free to ask.
Payment
We accept cash, personal checks and all major credit cards. Copay, coinsurance and/or deductable payments are to be paid at time service is rendered.
Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be charged a reduced session fee for the missed appointment. Details are in the Intake Packet.
